Our Patient Relations Representative is dedicated to assist patients with questions, concerns, compliments or suggestions about care/treatment received and to provide a way to manage conflicts in a respectful, supportive and timely manner.

You can provide feedback to the Patient Relations Representative directly in person, by telephone, email, or mail.

Monday to Friday, 8:00 am - 4:00 pm 

Office is closed on statutory holidays

For after hours assistance please leave a message by phone and the Patient Relations Representative will respond to you in a timely manner.
