The Health Records Transcription Professional is responsible for the daily qualitative and quantitative analysis of all charts to ensure clinical documentation is accurate and complete as per legislation and hospital policies. This position may provide transcription of dictated material and records, data quality reports, productivity reports, deficiencies and delinquency notifications ets.

Mandatory Requirements:
• Graduate of an approved Health Information or Medical Office program
• Must be able to type at a rate of 80 words per minute, 95% accuracy and data entry (6200 kph/95% accuracy)
• Proficiency in computer programs (Excel / Word / Meditech Expanse)
• Knowledge of medical terminology, pharmacology, anatomy

Responsibilities:
• Analyze and edits medical reports for patient’s records in accordance with established format and priority guidelines.
• Prepares patient-related correspondence, physician’s memoranda and reports as requested.
• Proof reads transcribed medical reports for accuracy and completeness; makes corrections or ensures any noted inconsistencies/omissions are clarified by the author.
• Forwards any paper documents for scanning on patient’s medical records which include inpatients, emergency department, clinics, SDS, radiology, certain physicians, echo cardiogram, stress tests and fax out reports.
• Other duties include sorting and distributing documents, answering phones and all other clerical duties as well as support for other team members within the Health Records department.
• Responds to inquiries regarding the medical record as required.
• Provides instruction to professional staff on the proper use and operation of hospital’s dictation system.

The Occupational Therapist is responsible for reviewing and prioritizing patient referrals; performing assessments and implementing therapeutic interventions; ensuring care and safety of patients during all patient interactions; delegation of tasks to OTA’s, PTA’s and RA’s in accordance to the College of Occupational Therapists of Ontario guidelines.

Responsibilities:
· Assesses and/or investigates all referrals received from assigned areas. Completes patient assessment using appropriate formats, outcome measures, and terminology according to the College of Occupational Therapists of Ontario’s Standards of Practice and Temiskaming Hospital standards.
· Establishes, plans, and implements patient programs. Ensures ongoing evaluation of patients and completes regular progress evaluations. Monitors effectiveness and progress of treatment program and decides when discharge is appropriate from occupational therapy.
· Accepts increases and alterations in caseloads and discharges appropriately.
· Interacts with other health care professionals, families, and significant others involved in the patient's care and ensures the most functional and beneficial patient program plan is devised and implemented. Educates patients and family members in the management of his/her concerns and physical and functional challenges.
· Ensures facilities and required materials are available and prepared for treatment times. Monitors patient programs to ensure a safe working environment is maintained and that program changes are initiated and carried out, as required.
· Completes all relevant documentation according to departmental standards set out by the College of Occupational Therapists of Ontario (COTO)’s standards for record keeping. Assures confidentiality of patient information in all interactions and documentation and follows the Public Hospitals Act and COTO regarding maintenance and flow of legal documents. Maintains relevant statistical information.
· Pursues new developments, trends and techniques and where appropriate incorporates these into patient programs to ensure quality patient care. Assumes and shares
responsibility for the education of volunteers and allied health professionals regarding Occupational Therapy mandates and treatment techniques.
· Reviews and revises procedures and methods of operation in conjunction with agreed upon improvement initiatives.
· In conjunction with other departmental employees, ensures departmental manual updates, audits, statistical information tabulation and departmental procedures are maintained so that the department meets its accreditation standards.

Qualifications
Graduate with a Masters in Speech and Language Pathology;
Current registration with the College of Audiologists and Speech Language Pathologists of Ontario;
